I've only booked through the cruise line, so I don't have any experience with booking on the spot with locals. However, I have read posts regarding excursions running late and from what I remember, if you booked through the cruise line, they may delay departure. If you booked on the spot, you might be stuck if it's running late and the ship leaves.

we always book through the cruise. peace of mind is worth the extra cost, especially if something goes sideways. we had an excursion bus break down once and the ship waited. wouldnt risk getting left behind for a cheaper rate.
