They do, but ours was barely cool—definitely request a real fridge from your steward.

Pretty sure it depends on the cruise line/ship. But like lantern-dawn said, I’m pretty sure you can request a mini fridge for medication. It’s probably something you want to do ahead of time by either calling the cruise line or submitting a request if they have it available via their website or emailed form. This way you don’t have to worry about it not being there ready for you. For example, this is the special needs form for celebrity, you’ll see on page 2 where there’s a fridge in stateroom option. https://www.celebritycruises.com/content/dam/celebrity/pdf/CEL-Special-Needs-Form-Revised-062221.pdf
On RCL and MSC there is a mini fridge in all of the Staterooms. On MSC it is pre-stocked as a minibar, but you can ask the Cabin Steward to clear it out for free. Now, in my experience, neither cruise line's mini fridges got very cold. They are at best coolers. If you can, ask for a medical fridge for your meds. If not, there are alternatives that you can purchase for any trip you make. We have one for our GLP-1 meds: https://4allfamily.com/collections/all/products/portable-medical-fridge-usb-insulin-medicines Not cheap, but TSA approved and USB powered.

We were on Mariner last year and had a balcony. The "fridge" is really just a cooler, and it actually warms up a bit when the room gets hot in the afternoon sun. If your meds are truly temp-sensitive, the medical fridge is the way to go. I'd also recommend bringing a small digital thermometer—we stuck one in ours and it was reading 65 degrees at one point. Not exactly chilled. Good luck sorting it out.

yeah the ones they call fridges are really just glorified coolers. we always bring a little insulated lunch bag and ask our steward for a bucket of ice daily. we keep the meds in the bag with the ice pack inside the cooler and that kept things properly chilled for us. saved us the hassle of getting a doctor's note for a special fridge.
